Job Description: Responsible for the coordination and management of patient caseload of cardiac patients. Performs the primary function of the nursing process--assessing planning, implementing, and evaluating care of all assigned patients. Collaborates with Care Coordinator and other members of the patient care team. Provides physicial and psychological support to patients, friends, and families. Provides home care nursing visits, as assigned, according to the policies and procedures and protocols of the agency. Makes nursing assessments and reports significant changes in patient condition to the Care Coordinator and/or patient care team members. As necessary, completes in-home admission and assessment visits within established parameters and develops/implements individualized patient plans of care. Utilizes the appropriate clinical pathways.
Experience Required: Must have experience in cardiac care and be proficient in cardiac care skills. Approximately one (1) year on-the-job experience in home care in order to acquire and effectively utilize knowledge of medical/surgical nursing. Knowledgeable of nursing assessment and evaluation skills.
Education: Current Pennsylvania State Registered Nursing license from an accredited school of nursing, Current Cardiopulmonary Resuscitation (CPR) Certification, Valid Pennsylvania driver licenses. Computer literacy is essential.
NOTE: This position is available for Full-Time, Part-Time, or Flex. Up to a $4,000.00 Retention Bonus. Flex staff will conduct assessment/consultative visits when the need is identified.
