Position Summary

The Director of Clinical Services is a registered professional nurse who has the authority and responsibility for directing and monitoring the activities of all assigned nursing personnel.  Under the guidance of the Executive Director of Home and Community Based Services, responsible for managing the clinical operations, efficiencies and quality home care outcomes through effective leadership and management of professional staff.   In the absence of the Executive Director, will be responsible for all implementation and administration of the Home Care program.

Essential Duties

•Ensures the Home Care department meets regulatory compliances
•Interprets standards, organizational policies and procedures to ensure compliance with external regulatory authorities and ensure that clinical direct report documentation meets internal standards. 
•In collaboration with the Director of  Client Services directs care/services provided
•Assists in facilitating the coordination of ancillary/paraprofessional services and the coordination of disciplines and utilization of community resources, including transition of care between institutions to a place of residence.
•Assists in the coordination of care/communication between team members, attending physicians, appropriate administrative staff, referral sources and case managers to ensure appropriateness of service, and facilitates any necessary changes to the plan of care.
•Reviews and interprets trends over time/data/client progress reports and provides concise findings to appropriate parties. 
•May be required to conduct the assessment and reassessment of clients, including updating care plans and interpreting client needs, while adhering to Organizational and Regulatory policies and procedures. Responsible for the hiring, management and supervision of home health and float pool professional staff including RNs, LPNs, and CNAs.   Responsible for team building, motivating, leading, hiring, training mentoring, directing and evaluating these individuals. Performs direct services for clients as needed including clinic oversight and medication setups.
•Responsible for the Quality Management Program for all areas of Home and Community Based Services.
•Markets program by calling on physicians, hospital or skilled nursing communities, discharge planners and case managers within an assigned territory to promote the appropriate service.
•Share in the responsibility for After hours on-call  backup for medical issues
•Other duties as assigned

Basic Qualifications & Experience

• Bachelors Degree in Nursing. 
• Colorado Registered Nursing License, in good standing.
• Minimum two years experience in home health
• Some experience preferred in skilled nursing and residential living
• Strong knowledge of governmental home health regulations, Medicare requirements preferred.
• Excellent assessment and communication skills required.
• One year experience supervising direct reports preferred.
• Proficient in Microsoft Office applications including Excel, Word  and Outlook
• Must be able to read, write and speak the English language.
• Must possess excellent customer service skills.
• Must have an acceptable Colorado driving record.

Working Conditions / Physical Requirements

•Light physical effort required by handling objects up to 20 pounds occasionally and/or up to 10 pounds frequently.
•Sits, stands, bends and moves intermittently during working hours.
•Is subject to frequent interruptions.
•Interacts with residents, family members, visitors frequently.
