Provides direct care services to patients and their families; conducts patients related activities; performs administrative and quality assurance tasks; establishes positive interpersonal relations; participates in community outreach, education and research activities; provides supervision as indicated or requested; handles unit specific duties and services as required; and performs miscellaneous other duties.
Master’s degree in Social Work from an accredited school of social work required; Licensed Social Worker required or Licensed Independent Social Worker. 18 months experience preferred; willing to cross-orient to both Home Care and Hospice; positive, mature, flexible, team player with strong clinical and organizational skills; provides psychosocial assessments, crisis intervention, grief counseling; bereavement and community linkage for home care and hospice patients and family; prior experience in healthcare preferred; must be willing to work flexible hours; must be physically and mentally able to perform all essential job functions.
