Utilize knowledge, skill, and practical experience to provide direct client/family care in the home; perform basic therapeutic and preventative nursing procedures in accordance with nursing standards, local, state and federal regulations, under the supervision of the Registered Nurse.

  • Prepare for visits by reviewing the Plan of Care and current client status reports; receive directions from RN Case Manager and/or supervisors as appropriate

  • Provide direct client care to clients assigned by the registered nurse and in accordance with physician's orders and the established plan of care

  • Identify factors affecting health care by evaluating the client's physical, mental, emotional, environmental and social needs with regard to clinical diagnosis and medical condition

  • Complete a nursing assessment on each client, including monitoring laboratory tests, and using other assessment tools to evaluate the client's status

  • Provide semi-complex treatments and/or nursing procedures within the scope of practice of a licensed practical nurse and agency policies

  • Administer medications and treatments as prescribed by the physician and monitor the client's response

  • Report significant client information and changes in the client's condition to the RN Case Manager and physician; document communication with the Case Manager after every visit

  • Assist the client with meeting ADL needs and facilitate the client's efforts towards independence

  • Promote a safe environment for the client

  • Responsible for increasing the client's awareness of medical condition and teach the prevention of illness and maintenance of good health

  • Maintain accurate and timely clinical records in compliance with internal, local, state and federal requirements

  • Use supplies and equipment in a cost effective manner

  • Maintain expected level of productivity per policy

  • Participate in weekend and holiday rotations and on-call duties as identified in Personnel policies

  • Participate in LHC (QI/QA) quality improvement process



  • Successful completion of an accredited Practical Nursing Program; current license through the State of Michigan State Board of Nursing as a Licensed Practical Nurse

  • Two (2) years nursing experience with current home health, medical, surgical or critical care experience preferred
