Cambridge Health Alliance has a unique and exciting opportunity for a Pharmacy Technician to work in a direct patient care role in our patients'' homes.
Duties and Responsibilities include:
1. Safely and accurately performs home POC and venipuncture INR testing of patients that require anticoagulation monitoring. 2. Ensures continuity of care for the patient by obtaining all necessary information and equipment prior to visits, contacting pharmacist or other care providers as indicated. Reports any outstanding contacts and additional follow-up required. 3. Manages home visit schedule, documentation, and coordination efficiently and effectively. 4. Documents activities effectively, accurately, and in a timely manner. Ensures compliance with the CHA policies and procedures. 5. Establishes effective relationships with patients and families, respecting patient rights and involving patients and families in decisions made as possible. 6. Triages patient care needs via telephone and home visit encounters to Clinical Pharmacist as appropriate. 7. Assists in the maintenance of various patient information lists (e.g., patient registries, patient follow-up, new patient referrals). 8. Performs outreach, follow-up, scheduling, and rescheduling of patients in the various pharmacotherapy services. 9. Assists in the processing, follow-up, monitoring and documentation of results of patient self testing (PST). 10. Complies with personnel record-keeping requirements (e.g., auto insurance documentation, licensure) on a timely basis. 11. Complies with all Home Visit Safety Guidelines as established by CHA Pharmacotherapy Services. 12. Complies with all CHA Pharmacotherapy Services Policies and Procedures for Environmental Safety, Quality Control and Equipment Management. 13. Communicates professionally and effectively with Pharmacotherapy Services staff, physicians, nurses, VNA''s, social workers, labs, front desk staff, patients and patient''s contacts, etc., while exhibiting and maintaining a high degree of responsibility for confidential matters 100% of the time. 14. Attends and participates in all required department meetings. 15. Demonstrates knowledge of pharmacology, drug therapy and current drug treatments in all areas of responsibility.
Education: High school diploma or G.E.D. certificate is required Associates or Bachelors degree preferred.
Registration: Must have a current pharmacy technician registration in good standing from the Massachusetts Board of Registration in Pharmacy, or be eligible for registration in Massachusetts.
Certification: Completion of a Board of Pharmacy approved certifying body pharmacy technician certification examination or successful completion of said examination within first year of employment.
Work Experience: Minimum of three (3) years of experience as a pharmacy technician is required. In addition, a demonstrated administrative competency is required and direct patient care experience is preferred. Ability to perform various skilled functions such as blood draws (when appropriate) and operate CHA-approved POC INR machines. Must be able to with stand extended periods of sitting or standing. Ability to enter data into the computer.
Proficiency in various database applications, all Microsoft Office Suite products, and other software products as required. Knowledge of standard office practices and procedures. Ability to prepare a variety of statistical, financial, analytical and narrative reports as required. Ability to effectively prioritize responsibilities while under pressure. Ability to work independently in a complex, demanding, busy, stressful environment. Ability to correctly and efficiently document and communicate patient information. Ability to process sensitive information in a timely manner and maintain strict confidentiality of information assembled in the performance of daily responsibilities. Work is performed in clients'' homes with the employee driving from location to location during the work period. The driving requirements include in-town and highway driving in both good and inclement weather.
