San Diego Home Health is seeking a part-time Certified Home Health Aide. The HHA works in an environment that calls for flexibility in serving patients of diverse socio-economic and cultural backgrounds. This position is expected to perform all of the duties related to a home health aide in a home environment. Must be a current California Certified Home Health Aide (CHHA).
POSITION SUMMARY: The Home Health Aide is expected to perform all of the duties related to a home health aide in a home environment, and requires the knowledge and ability to perform as a health care professional. Duties include care of patient’s ADL needs as well as other duties as assigned by the supervising clinician.
KEY JOB FUNCTION
- Receive and processes referrals for Supervising Clinician.
Observes and reports changes in client’s condition to Supervising Clinician.
Assists patients to improve in ADL’s (Activities of Daily Living).
Communicates patient’s supply needs to Supervising Clinician or medical staff.
- Attends In-Service Training programs to maintain currency in practice.
- Maintains confidentiality and does not discuss patient or patient’s information outside of the Agency.
- Current certification as a Certified Home Health Aide by the State of California required.
- Valid driver's license and reliable transportation.
