Looking for a Case Manager who oversees the provision of services and supports to individuals in accordance with each individual’s identified outcomes as well as The Arc’s Mission Statement and Guiding Values. This includes developing each person’s Habilitation/Treatment Plan, assuring all documentation of service provision, and assuring that services meet the goals and objectives as outlined in each planning document. Case managers submit required documentation, including Medicaid billing, complete monthly, quarterly, and annual documentation and visits/contacts. Other duties are visits each individual on the assigned caseload at least one time per month. This position creates, monitors and renews Core Service, Grant, and General funding packages, participates in creating and monitoring MRDD Waivers and Treatment Plans. The Case Manager is accountable to the person receiving supports, their team, and the agency.
Bachelors’ degree in human services preferred with a desired concentration of study related to developmental disabilities/mental health and/or social work. Two years’ previous social services work experience with case management responsibilities, including one year in the field of developmental disabilities. Any equivalent combination of related education and experience might substitute for a degree requirement in addition to the two years of social service work experience required above. Life experience with individuals with developmental disabilities/mental health may be considered in place of social services work experience requirement on a case-by-case basis. Vehicle, current Alaska driver’s license, required level of automobile insurance, background check and TB test. Must maintain current certifications in First Aid, CPR, and Mandt training. Other training may be required. An Arc application must be submitted to be considered for any position. Excellent benefits package. DOE.
