The Position:  The Hospice Clinical Nurse Manager is responsible and accountable for coordinating the activities of a hospice interdisciplinary team and providing clinical and managerial leadership.  Directs all aspects of the interdisciplinary care and services provided to our patients in their homes. Actively participates in the achievement of clinical and financial outcomes associated with the care provided.  Provides education and support to the team and monitors care delivery to ensure compliance.
Responsibilities:

*Supervise, evaluate and coordinate the various members of the interdisciplinary team

*Ensures compliance with all federal, state and local government regulations and accreditation                                                                                                     standards.

*Assures continuity of care
*Serves as a patient advocate
*Develops and manages team budget
Requirements:
*RN with BSN preferred
*3-5 years supervisory or leadership experience in a healthcare setting
*Hospice case management experience preferred
*Demonstrated commitment to serving others
*Passion for focusing on hospice care
*Record of professional growth and advancement

 

The desired candidate will also possess excellent interpersonal, organizational, and communication skills as well as the ability to problem solve and negotiate. Must have established analytic and strategic planning abilities, and have demonstrated abilities to effectively coach employees and influence change.
