About the role.
As a liaison, this person is responsible for managing leads in primary referral hospitals and ensuring a smooth discharge process for patients transitioning from a hospital/clinic setting to their home, where they will use our home infusion services. Additionally, the nurse liaison will develop strong relationships with the key referral hospital personnel and identify new opportunities for partnering and growth.
As a RN Clinician this person provides direct nursing services and case management related to infusion therapy for patients in their home, hospital, clinic or an alternate living site. This is accomplished through collaboration with the patient and/or caregiver, the physician, other health care providers and the payer. This position functions as a professional role model, demonstrating competency in all phases of the nursing process, collaborating with other health care workers to promote quality patient care, and acting as a resource for home care nursing staff for patients with infusion home care needs.
About Infusion Therapy.
Home infusion provides an alternative to receiving intravenous (IV) therapy in the hospital. Home Infusion Therapy is the intravenous administration of medication to patients in their own home.
This position is part of Providence Infusion and Pharmacy Services. The Nurse Liaison's office will be in Everett, WA, but this individual will be expected to travel Snohomish County. This is a full time (40 hours per week), day shift. Position schedule includes rotating Saturdays.
Licenses/Certifications: Current RN license in Washington State. Valid driver�s license and automobile insurance. Current CPR certification.
Experience: One year of experience in acute care nursing. Home health or home infusion experience preferred. Prior sales experience preferred.
Other knowledge: Must know PICC lines, must be able to access ports, central lines and dressing changes.
