Serves as the leader of the patient care team. Assesses patients and establishes a plan of care engaging the patient and/or family in the process. Implements the plan of care, including patient and family education, in collaboration with other members of the team. Coordinates the patient's care with other disciplines and within the nursing care team. Clearly communicates expectations to other members of the nursing care team, delegates tasks appropriately, and follows up to ensure that tasks are completed. Assesses the patients response to the plan of care, informs MD of changes in the patient's condition, and alters the plan of care as needed. Initiates discharge planning upon admission utilizing appropriate resources. Responsible for skilled nursing care to patients at home under the direction of and in accordance with physician's plan. Serves as a liaison between Community Medical Centers and the community by initiating, intervening, and evaluating nursing services to patients in their homes.
EXPERIENCE AND EDUCATION
MINIMUM REQUIRED: Graduation from a School of Nursing or successful completion of sufficient units in Nursing to qualify for state licensure. Approximately one year acute care experience.
MINIMUM REQUIRED: Current California Registered Nurse License. Current BLS certification. Valid California Class III driver's license, proof of auto insurance and clean driving record. Dependable transportation. The Central Valley offers some of the lowest housing rates in the California. Community Regional Medical Center has the only perinatology program in the Central Valley and receives patients from 15 other area hospitals because of its expertise in handling high-risk pregnancies and deliveries. There is a neonatologist on call 24 hours a day, seven days a week, as well as a respiratory therapist and trained newborn care staff available to care for high-risk babies. We deliver more than 10,000 babies a year and operate the only birth center in the region designated to handle high-risk pregnancies.
