NATURE AND SCOPE OF POSITION: In accordance with the stated Mission of Northland Hospice and Palliative Care and its Policies and Procedures, the Clinical Director is responsible to direct and coordinate all patient care services, provide nursing expertise in the development of policies and programs and serve as a representative of the nursing staff to outside agencies and the community. The Clinical Director guides the Interdisciplinary Team to facilitate the continuity of the physical, emotional, and spiritual patient care according to the individual patient’s Plan of Care. The Clinical Director is supportive to, and supported by Northland Hospice and Palliative Care staff and volunteers. ESSENTIAL DUTIES • Makes patient assessments, teaches, supervises, and counsels the patient, family members and staff regarding the care and other related standards. • Negotiates patient insurance coverage with appropriate insurance personnel (i.e. case manager) • Assigns nursing and home care staff to patient/family • Supervises and evaluates the care provided by nursing staff, paid and non-paid, and allied health personnel for conformity to Northland Hospice and Palliative Care Policies, Procedures and Hospice Standards of Care • Develops and documents written material related to hospice care • Participates in the employments decisions and development of job descriptions related to the nursing staff • Provides orientation for nursing staff • Augments the services provided by other team members by: o Participating in the formulation of the patient Plan of Care o Facilitating regularly scheduled case conferences involving Inter-Disciplinary Care Team o Planning and facilitating regularly scheduled in-service opportunities • Facilitates nursing staff meetings and participates in regularly scheduled administrative meetings • Audits patients charts and assigns nursing staff to perform peer review audits • Serves as a Northland Hospice and Palliative Care nursing representative for outside organizations and institutions • Facilitates marketing of Hospice service to physicians and medical community • Serves as Liaison between patient, family, primary physician and entire hospice team regarding the therapeutic plan as prescribed by the physician • Develops and supervises hospice home practicum program and appropriate orientation • Works with the Human Resources Manager to ensure that clinical staff annually verifies current status of nursing license, liability insurance coverage and TB screening • Coordinates on-call schedule • Provides consultation and assistance, as needed, in the coordination of final 24 hour care • Contributes nursing expertise for policy development, patient care, volunteer evaluations, and annual agency quality assurance evaluation • Participates in discharge planning for Hospice patients from any care facility • Assumes compliance with State and Medicare regulations In Facilitating the Hospice concept of service, the Clinical Director is responsible to: • Demonstrate sensitivity to individual personalities and need systems of team members • Demonstrate an understanding for the expertise and comfort level of the nursing staff • Facilitate constructive communication between members of the Interdisciplinary Care Team • Monitor level of participation of patient care volunteers, facilitating their comfort and acceptance by professional staff • Participate in direct, productive communication with team members when problems arise • Serve as Executive Director in his absence or as directed • Maintain compliance with HIPAA requirements as per organization policies EDUCATION AND/OR EXPERIENCE • Bachelor of Science in Nursing required • Master of Science in Nursing or Health related field preferred • The Clinical Director shall be a registered nurse credentialed by an accredited school of nursing, with a minimum of two (2) years experience, preferably in supervision of community, and/or hospice or home care • Case management and marketing experience required • Previous one (1) to two (2) years leadership/management experience • Knowledge of budgeting, hospice and Medicare requirements • Graduate of Northland Hospice and Palliative Care Volunteer Training Program KEY COMPETENCIES • Self-starter, proactive, works quickly and accurately • Adaptability • Excellent communication and teambuilding skills • Excellent interdisciplinary teambuilding skills • Must be able to read written instruction and comprehend verbal instructions given in English • Demonstrate compassion for individuals in the dying process • Demonstrate sensitivity to needs of patients/families in stressful situations and tolerance for individual family life styles • Demonstrates sensitivity and respect to colleagues • Organizational and supervisory skills required • Demonstrate good negotiating skills • Demonstrate a clear understanding of the hospice concept and the ability to convey it in a practical way to others CERTIFICATES/LICENSES/REGISTRATIONS • Reliable transportation, current driver’s license, and current auto insurance • Possess a current and valid Fingerprint Clearance Card • Current and valid American Heart Association CPR Certification • Possess a current license as a Registered Nurse in the state of Arizona
