ADMIRAL HOSPICE CARE, INC. is a rapidly growing hospice organization covering areas in the Los Angeles and Orange County communities. The agency has immediate openings for experienced and qualified candidates.
The Admitting RN/Case Manager is responsible for managing a caseload of patients assigned within a specific geographic area. The Admitting RN/Case Manager will oversee care while working with a team which may include a health, chaplain, social worker, medical director and others to allow for the best of care for the patient and family.
The Admitting RN/Case Manager’s primary responsibility is to present Admiral Hospice Care services and philosophy to patients and families in a manner that results in the admission of appropriate patients. The process may include ordering DMS, profiling medications, hospital discharge and follow-up tuck-in.
Current RN License of 2 year or more duration.
Minimum of 1 year recent experience as an RN in a hospice environment.
On-call availability a must.
Certified Hospice & Palliative Care Nurse (CHPN) preferred.
Demonstrated supervisory/management experience.
Demonstrated success with an interdisciplinary team.
Computer literate & experience with Consolo software a plus.
Proficiency in IV pumps is preferred.
Bilingual Spanish a plus.
Due to county contracts candidate must be able to supply proof of immunizations in addition to normal health requirements and credentials. Candidates must also have strong verbal and written communication skills, be willing and able to drive to referral and/or patient locations assigned within your geographic territory. The successful candidate will also have critical thinking skills, ability to make decisions, strong organization and time-management skills.
