|Responsibilities:||PCP is a non-exempt service position responsbile for providing homemaking and personal care services to clients in their homes. Accomplishes job duties in a timely, safe, and professional manner. Procedures which may be required of the personal care provider include (but are not limited to) any combination of the following: performing housekeeping tasks as directed; making beds, changing and laundering linens; preparing meals and assisting with eating as required; grocery shopping; cleaning stove; defrosting refrigerator; accompanying patient to physician or clinic via taxi or client's care; vacuuming and dusting; cleaning bathrooms; assisting with personal care such as bathing, hair care, assistance with dressing, and routine skin care; assisting with toileting such as ambulation to and from bathroom, assistance with relation to bowel and bladder, and bedpan or urinal routines; assisting with simple ambulation. All procedures are carried out in accordance with universal precautions. Assures client's satisfaction with care received, by meeting client needs within reason and consistent with the goals of the program, and by advising supervisor of requests which cannot be met or of conditions which are unsafe. Acts as a dependable caregiver by providing services when scheduled, communicating promptly to Agency and client any schedule change, and completing and submitting required paperwork accurately and on time.|
|Minimum Qualifications:||Graduation from high school or equivalent preferred but not required. Evidence of twenty hours of training as a personal care provider required. This may be from a hospital, nursing home, or home health facility. Twenty hours of formal training may be waived if the employee has had a job history or job training where personal care skills can be validated by documentation or on-site observation by a clinical supervisor. Complete knowledge of universal precautions. Sufficient knowledge of personal care practices and procedures to follow accepted. Frequent exertion of 25 pounds of force, and occasional exertion of 50 pounds of force. practices.|

Medical and non-medical home care services are available on an intermittent basis 24 hours a day, 7 days a week.
Corporate and community wellness screenings and immunizations, including foot care services for seniors and flu shots, are available by appointment on an individual or group clinic basis.
